6. On the front of the envelope with a potential supplier must indicate:
1) the full name and postal address of potential supplier;
2) the name and postal address of JSC " EK REC ", which should correspond to the information specified in the announcement of the purchase ;
3) the name of purchases of goods for participation in which price proposal .
7. The envelopes with the potential supplier is held at 14:00 "14" December 2012 , available at : 070002 , Ust - Kamenogorsk city . Bazhov , 10 cab. 411 ( conference room ) .
8. Pricing shall include all costs associated with the delivery of the goods .
9. The project procurement contract , indicating the essential conditions attached.
10 . Procurement contract is for not more than twenty (20 ) calendar days from the date of signing of the protocol on the outcome .
11. Each potential supplier is only one quotation , signed and sealed ( if any) of the potential supplier, which shall contain the following information and documents:
1) the name , physical address potential supplier;
2) the name , characteristics and quantity of goods supplied ;
3) the place and date of delivery of the goods;
4) The unit price and total price of the goods excluding VAT, with the inclusion of costs associated with the delivery of goods;
5 ) A copy of the certificate of state registration certificate;
6 ) A copy of the license ( if the terms of the procurement activity is expected , which is subject to compulsory licensing) .
7 ) the technical specification, signed by the potential supplier ( if the announcement of procurement price proposals request containing technical specifications and the requirement to provide a potential provider of technical specifications) ;
8 ) a notarized copy of the certificate of origin ( Form CT KZ) ( if the purchase price proposals request are carried out in accordance with subparagraph 1) of paragraph 93 of the Rules of the Fund purchases )
12. Any interested person has the right to be present at the opening . Permission is granted to audio and video recording of Bid Opening , except as required by law .
Authorized representatives of potential suppliers have the right to see the price offers of other potential providers by hand .
